We’re thrilled to announce our exciting new project, Help Home from Hospital, funded by Nationwide Building Society for two years starting April 2021.
Many people with complex emotional, social and physical support needs are discharged from Bournemouth Hospital after emergency medical treatment into inadequate or unsuitable housing, with little or no support to enable them to recover and reintegrate in their local community. Many live in extreme financial hardship and are at risk or already homeless due to chaotic lifestyles resulting from addiction and/or mental health issues.
The project will focus on supporting vulnerable patients due for discharge from the Acute Medical Unit at Bournemouth Hospital who have inadequate housing or are at risk of homelessness to deal with immediate housing crisis, then advocate and guide them to deal with other personal hardship crises so that they become more resilient and able to self-manage robust and sustainable independent living strategies.
A huge thank you to Nationwide for funding this project which will enable us to refocus our Help Through Crisis project to provide high-quality advice and advocacy to people being discharged from hospital into the community who are facing poverty, debt and housing crises whilst still in poor health and struggling to get the support and welfare benefits to which they are entitled.
Help Through Crisis is our five year Big Lottery Funded project providing advocacy and case work support to disabled people facing extreme financial/housing hardship which is due to end in June 2021.
